試験登録（ランダム化）した患者が転院することになった。どのような手順が必要か？

If the patient is transferred to another hospital that is participating in REMAP-CAP, please go to the patient summary page (as pictured in your screenshot) and click “transfer patient”. You will then be asked to select the site you are transferring the patient to, and the date and time that the patient was transferred.

The receiving site will then have to confirm the patient transfer, and the patient will then appear on their patient list. They will then continue to collect data for this patient. Note that this is considered the same admission, and if the patient is transferred directly from ICU to ICU then this is considered the same ICU admission.

If the patient is transferred to another hospital that is not participating in REMAP-CAP, then this is considered a discharge from the randomising hospital. You would enter the date and time of transfer as the date and time of hospital discharge (and ICU discharge if transferred from ICU). The hospital discharge destination is “transferred to another acute hospital”, and you will be asked to enter details relating to the subsequent admission at the receiving hospital.
