REMAP-CAPに登録後、転院した症例において、Discharge は《どちらの》退院を指すか？

Data Completion Guidelines V10 に、以下の記載があります。（p.12-13）

Patients transferred to a non-REMAP-CAP participating hospital
If a patient is transferred to a hospital not participating in REMAP-CAP it is the responsibility of the randomizing hospital to complete all data entry for the patient. Data collection stops at time of discharge from your hospital, you will only be asked to collect ultimate hospital discharge date.

ここで、your hospital は REMAP-CAP 参加病院のことだと思います。したがって、REMAP-CAP 参加病院を離れた後は、Daily ほかのデータ入力は不要ということになります。

一方、”Day 21″ の “Discharged from hospital” の選択により、”Form 6: Discharge” の入力が求められます。この “Form 6: Discharge” に “3. Hospital Discharge” – “Destination” – “Transfer to another acute hospital” (Data Completion Guidelines V10 p. 159) という選択肢があることから、やはりこの Discharge は、あくまで REMAP-CAP 参加病院からの退院（または転院）と解釈できるのではないでしょうか。

これは、入院日も、REMAP-CAP 参加病院への入院日ではなく、当該疾病による最初の入院日とする、という方針と矛盾するようですが、上記の “Transfer to another acute hospital” を選択すると、”Ultimate hospital discharge date” および “Status at ultimate hospital” (Alive/Deceasesd/Unable to ascertain) の入力を求められることから、

“hospital discharge” = REMAP-CAP 参加病院からの退院
“ultimate hospital discharge” = 今回のエピソードで最後に（90日で打ち切り）入院した急性期病院からの退院

と解釈できるのではないでしょうか。
